The Community Habilitation Program is designed for children and adults with intellectual and developmental disabilities (IDD) who want to strengthen the skills needed for greater independence at home and in their communities. Serving individuals throughout the five boroughs of New York City, the program provides individualized skills training based on each person’s unique needs, interests, and Life Plan. Services are delivered in a person-centered environment through a collaborative partnership between the individual, their family, and Heritage Human Services, Inc. staff to support meaningful skill development in everyday settings.
Core Focus Areas
The Community Habilitation Program provides individualized training and support to help participants build, maintain, and improve essential life skills. Community Habilitation Specialists work closely with each individual to achieve their valued outcomes identified in their Staff Action Plan based on their Life Plan.
Daily Living Skills
Instruction in personal hygiene, housekeeping, laundry, meal preparation, and other everyday household tasks.
Financial Literacy
Training in money management, basic math, budgeting, banking, and responsible financial decision-making.
Community Mobility
Comprehensive travel training that helps individuals safely navigate public transportation and access community resources.
Socialization & Community Inclusion
Opportunities to strengthen social skills, build relationships, and participate in recreational and community activities.
Self-Advocacy
Support in making informed choices, understanding individual rights, communicating effectively, and developing greater self-confidence.

One-on-one or up to one-to-three support based on the individual’s needs and goals.

The program emphasizes education and practical skill-building that promotes active independence at home and in the community.
Eligibility Requirements
The Community Habilitation Program serves children and adults diagnosed with intellectual or developmental disabilities who live with their families or independently within the community.
Additional eligibility information includes:
- Medicaid-funded through Home and Community-Based Services (HCBS) Waiver programs
- No out-of-pocket costs for eligible participants and families
- Eligibility requirements are the same as those applicable to all HCBS Waiver services
What to Expect
Community Habilitation services are provided in the individual’s home and community through individualized, goal-oriented instruction that reflects their Life Plan and personal objectives. Services are scheduled collaboratively with the participant and family to provide flexibility while supporting the development of practical skills that encourage greater independence in everyday life.
