To enroll in Heritage Day Habilitation Programs: Group Supplemental Program, Community Pre Vocational Program, Community Habilitation Program, Respite Program, Behavior Management Training, Individuals Skills, or any other program, please submit the following:
- OPWDD Approval Letter for the program
- Life Plan with Heritage listed as the program Provider
- Level Of CARE – Updated LOC
- Evaluations: Psychological and psychosocial
- Physical examination with PPD test result
- If available the most current Individualized Educational Plan (IEP)
